PREFERRED COMPANIONS HEALTH GUARANTEE/CONTRACT Buyer Name: ______________________________________________ Address: _________________________________________________ Phone: ________________Email: _____________________________ Description of Dog Breed _________________________Sex/Markings_____________________________ Whelped______________ of Sire and Dam____________________________________ Puppy has been wormed and vaccinated, with Progard 5  on____________ Buyers Responsibilities > Have the puppy, examined by a vet of their choosing, within 3 Working Days of pick-up of the puppy.  If the puppy is deemed not fit for sale at that time, the Buyer must notify me immediately, by phone or email.  Failure to do this, will result in cancellation of the guarantee. Ø      Provide puppy with routine heath care, such as but not limited to: o       Annual physical exams o       Follow veterinary guidelines for vaccinations and worming o       Start puppy on Heart Worm Prevention, with the first vet visit Puppy must be paid in full by 8 weeks of age, or all deposits and payments, and all claims to the puppy will be forfeited.  All deposits are non-refundable. Health Guarantee Ø      This puppy is guaranteed for 1 year against fatal and life altering congenital disease, which adversely affects the health of the animal.  This includes severe Hip Dysplasia, Kidney, Heart, Pancreas, Spine, and Liver problems of a severe life threatening, or altering nature.  If the dog dies with this One Year time frame, the Buyer agrees to have an autopsy to determine the reason.  If the reason is congenital, the Seller will replace the puppy. Ø      This guarantee does not cover what is considered normal in the Bulldog & Pug Breed, Stenotic Nares (collapsed nostrils), Elongated Soft Palate, Entropion (eyelid inward rolling), Luxated Patella’s or Skin Allergies. Ø      All Items covered under this guarantee are by replacement only.  We do not give refunds. Ø      Seller will not be responsible for any medical expenses incurred.  Buyer is aware that owning a puppy means trips to the Veterinarian, and understands that he/she will be responsible for all payments. Ø      As of the date of delivery, this puppy has received proper vaccinations and worming appropriate for it’s age.  The buyer has been informed of these, and agrees to follow further recommendations from her Vet. Ø      This guarantee covers Parvo Virus, for the period of 3 days from pickup of the puppy.  This guarantee does Not include Coccidia and Kennel Cough, which is common, in puppies that are shipped, and is usually caused by bacteria. Ø      It does Not include Hypoglycemia, (common in small breeds of dogs like Chihuahua’s) which can be prevented by ensuring that the puppy is eating and drinking properly and often.  Giving a teaspoon of pancake syrup by mouth, if puppy is exhibiting symptoms can treat hypoglycemia.  Contact your vet for further recommendations. Ø      A puppy showing serious signs of any genetic or congenital defect will be replaced as follows: o       Animal must show signs of the defect by One Year of age.  Seller must be notified within 2 Business Days of the licensed Veterinarian’s determination.  This diagnosis must also be documented by a second, Vet, independent of the first Vet, or his associates. o       A puppy with a defect must be returned or euthanized with prior permission of the Seller to qualify for a replacement puppy. o       A returned puppy must be returned to the seller in good condition, other that the specified problem, at the buyer’s expense. All paperwork, including Registration Papers, must be returned with the puppy.  The owner will then qualify for a replacement puppy. o       This guarantee applies to the original puppy, and purchaser, and is not transferable to a second party. o       Replacement puppy will be mutually agreed upon by both parties, and will be of comparable quality.  It is understood that the replacement puppy may not be from the exact parents of the original puppy. o       There are certain disorders that may be caused by environmental factors, for example allergies, thyroid dysfunction, Demodectic mange and other autoimmune disorders that are not covered by this guarantee. o       Special concerns with this puppy are as follows: ____________________________________________________________________________________________________________ ______________________ o       Any physical problem, which is or could be the result of injury, is not covered under this guarantee. o       There is no guarantee or warranty that the Buyer will be able to Show or Breed the puppy regardless of show or breeding potentials. o       This contract supersedes any and all previous agreements. o       This contract is null and void if the terms and conditions are not met. o       Buyer agrees that he/she understands this contract fully, and that this contract is under the jurisdiction of the State of Minnesota, in the County of Beltrami.  It is agreed that the place of venue shall be in Beltrami County. Shipping and Delivery Ø      Puppy will be shipped between the ages of 8 and 10 weeks.  Most will be shipped at 8 weeks old, but some need more time, and will ship at a later date.   Weather conditions must be right, in order for the puppy to ship.  Please check guidelines with Northwest Airlines, our Local Airline.  Puppy is vet checked before flight, and Buyer assumes all care and responsibility of the puppy from flight take off.
